June 16

• S&G, 801 W. Main St., Troy — Corrected During Inspection; Critical: Handwashing sink not accessible. Handwashing sink in back area completely inaccessible due to various maintenance items being stored in front of it. Handwashing sink shall always be accessible for employee handwashing.

Critical; Repeat: Food equipment surfaces not cleaned at required frequency. Observed build up of mold-like substance on the interior of the ice machine. Discussed ice machine shall be cleaned per manufacturer’s specifications or as often as necessary to prevent a build-up.

Repeat: Food not protected from contamination. Walk-in cooler fans observed with build-up of dust and dirt at the time of inspection.

Warewashing equipment and/or components not cleaned at required frequency. Observed 3-comp to be unclean and supplied with several inadequate items at the time of inspection hindering facilitation of appropriate cleaning.

Repeat: Non-food contact surface(s) not cleaned at the required frequency. Observed severe ice build-up on top of reach-in freezer in retail sales area. De-ice and clean as often as necessary to prevent build-up of debris and residues such as ice.

Repeat: Backflow prevention device not provided on carbonator. Backflow prevention device shall be installed on soda beverage carborator

Critical; Repeat: Insufficient air gap between the flood rim level of drain from ice bin. Ice bin drain must have at least a 2-4 inch air-gap from flood rim level of drain to prevent back siphonage from entering ice bin.

Critical; Repeat: Improper backflow prevention device installed on a water supply system. Beverage machines shall be equipped with an ASSE 1022 dual check backflow prevention device.

Maintenance tools improperly stored. Observed maintanence tools stored in back area next to and in front of hand sink. Maintenance tools shall be stored separately and away from areas needed and intended for normal operation of the retail food establishment.

Repeat: Facility not maintained clean. Observed build up of dust, dirt and other debris on floors underneath 3-comp sink, underneath ice machine, and throughout facility at the time of inspection. Physical facilities shall be cleaned as often as necessary to prevent build-up of dust, dirt and other debris.

Repeat: Floor and wall junctures not properly coved or closed and/or floor drains not provided. No coving observed underneath section of 3-compartment sink fixture.

Repeat: Floors, walls, and/or ceilings not smooth and easily cleanable. Observed area of floor underneath 3-comp sink drainboard used for air-drying without proper floor tiling. Floors shall be smooth and easily cleanable.

• Overfield Early Childhood Program, 172 S. Ridge Ave., Troy — No violations were documented at the time of inspection.

June 17

• Big Mike’s Gas N Go LLC, 1333 Archer Drive, Troy — Facility does not have an employee with manager certification in food protection. Manager’s Certificate provided during pre-inspection not on-site at the time of inspection. Please keep certificate on file for all future inspections.

Corrected During Inspection; Critical: Handwashing sink is being used for purposes other than handwashing. Observed coffee stains in handwashing sink on self-serve beverage line of retail sales floor. Discussed handwashing sinks shall be used for no other purpose other than handwashing.

A food packaged in the facility not labeled properly. Observed cinnamon rolls from approved source not properly labeled with all required information. Discussed labels shall be prominently labeled with all required information including the place of the distrubutor/packer of the food item.

Food not protected from environmental sources of contamination during preparation. Observed stainless steel prep table without proper/adequate space for preparation of hot holding food items for the retail floor. Discussed food must be prepped on a clean and sanitized surface before packaging or transporting to retail floor.

Warewashing equipment and/or components not cleaned at required frequency. Observed 3-compartment bays to be dirty at the time of inspection. Warewashing facilities shall be cleaned as often as necessary to keep them clean.

Non-food contact surfaces of equipment are unclean. Observed build-up in microwave oven underneath retail self-serve area. Non-food contact surfaces shall be clean to sight and touch.

Mops dried improperly. Observed mop air-drying on the ground next to mop basin in back of house. Discussed mops shall be hung to air-dry to prevent contamination of physical facilities’ floors and walls.

• Schwan’s Home Service Truck 517217, 2991 S. County Road 25A, Troy — No violations were documented at the time of inspection.

June 18

• Marion’s Piazza, 1270 Experiment Farm Road, Troy — Corrected During Inspection; Critical: Handwashing sink is being used for purposes other than handwashing. Observed debris in hand sink. Discussed with PIC that handwashing sinks shall be used only for handwashing.

Utensils, thermometers, and/or pressure gauges not in good repair or calibrated. Observed thermometer not calibrated properly. Discussed thermometer calibration with PIC at time of inspection.

Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed deep scoring of cutting board surface for salads. PIC called and placed order for new cutting surface at the time of inspection.

June 19

• Troy Food Mart Inc., 201 Staunton Road, Troy — At the time of inspection, employees unable to test manual warewashing sanitizer solution.

• Subway, 998 N. Market St., Troy — Repeat: Non-food contact surfaces of equipment are unclean. Observed buildup underneath shelves in walk-in cooler. Observed unclean floors with food residue and liquid.

Repeat: Non-food contact surfaces of equipment are unclean. Observed underneath tea dispensers in cabinets, spills that have become sticky. Cabinet handles are sticky to touch.

Repeat: Facility not maintained clean. Observed food and beverage spills throughout facility. Observed beverage spills under bag and box soda storage. Observed buildup on floors throughout kitchen and prep area. Increase cleaning frequency on all floors throughout.

• Piqua Country Club Snack Bar, 9812 Country Club Road, Piqua — Corrected During Inspection No towels or drying device at the handwashing sink. No hand towels provided at employee hand sink upon entering the kitchen area for inspection. Hand sinks shall always be adequately stocked to facilitate proper handwashing for food employees.

June 22

• Poppin’ Off, 125 S. Market St., Troy — No written procedures for responding to vomiting or diarrheal events. At time of inspection a copy of the body fluid clean up procedure was not available.

Utensils and equipment contacting non-TCS foods not cleaned at required frequency. Observed underside of grate coming from the Cornado (corn popper) unclean. Observed food residue and build-up on the underside of the grate.

Repeat: A food packaged in the facility not labeled. Observed various popcorns with different toppings, packaged in facility, out for customer service, that were not properly labeled.

• Subway, 982 W. Main St., Tipp City — Non-food contact surfaces of equipment are unclean. Observed reach in coolers below microwave are not clean to sight and touch.

• Homestead Golf Course Pop House, 5327 Worley Road, Tipp City — Repeat: Facility does not have an employee with manager certification in food protection. No manager certified individual in food protection observed for facility at the time of inspection.

Repeat: FSO did not have a person in charge per shift with the person in charge certification in food protection. No level 1 person-in-charge certified as a food handler observed at the time of inspection.

June 23

• ReU Juicery & Organic Kitchen, 5 E. Main St., Troy — Corrected During Inspection; Critical: At the time of inspection, the bottles of juices in the dinning area reach-in cooler were observed with improper health claims. Upon making the PIC aware of this, the PIC began the process of removing the health claims from each bottle.

The multi-use serving and prep utensils in the back kitchen area were observed being stored with the food contact surfaces facing up. To prevent contamination of food contact surfaces, store the utensils with the handles up.

June 24

• Botston Stoker, 1293 S. Dorset Road, Troy — Water treatment device or backflow preventer not inspected/serviced with documentation as required. Observed back flow device is past due for inspection. Please schedule inspection with a licensed plumber and update tag.

Physical facilities not maintained in good repair. Observed damaged/missing cove base under 3 compartment sink. Repair or replace.

• Budget Inn of Troy, 1330 Archer Drive, Troy — No violations were documented at the time of inspection.

• Ducky’s, 100 W. Market St., Troy — Critical; Repeat: Presence of live insects, rodents, and other pests. Observed rodent droppings in back room with freezers. Unable to identify if there is a current or past pest issue. Discussed approved wet cleaning methods with PIC.

• Rudy’s Dairy Bar, 1125 S. Miami St., West Milton — No violations were documented at the time of inspection.

• Old Mason Winery and Vineyard Inc., 4199 S. Iddings Road, West Milton — No violations were documented at the time of inspection.

June 25

• Troy BK Rootbeer Inc., 2780 Stonecircle Drive, Troy — Repeat: Outer opening not protected. Observed gap in door sweep on back door near all reach in coolers and freezers. Ensure light is not visible through door sweep to eliminate pest entry point.

Repeat: FSO did not have a person in charge per shift with the person in charge certification in food protection. Discussed at least one person per shift must have person-in-charge food handler certification.

• Fulton Farms Market, 2393 State Route 202, Troy — Critical; Repeat: Missing and/or incomplete allergen declaration(s). Observed fish removed from original packaging without proper labeling. Confirmed facility removed fish from sale.

• Culver’s, 2100 W. Main St., Troy — Repeat: Improper use or placement of insect control devices. Observed glue traps in back storage area, not in enclosed boxes. All pest control devices must be in enclosed containers to prevent contamination.

June 26

• West Milton Stopo & Go, 1046 S. Miami St., West Milton — Repeat: Facility does not have an employee with manager certification in food protection. Observed no employee with Manager’s certification in Food Protection.

Critical; Repeat: Employees are not informed in a verifiable manner of their responsibility to report information about their health. No employee health policy kept and retained on-site at the time of inspection.

Repeat: No written procedures for responding to vomiting or diarrheal events. No bodily fluid clean-up procedures kept on-site for responding to bodily fluid accidents in the retail food establishment.

Critical; Repeat: No air gap or approved backflow prevention device on the plumbing system. Observed drink beverage machines connected to water without proper backflow devices in self-service / retail station.

Critical; Repeat: A cross-connection was observed attached to the three compartment sink fixture which was lying in the rinse compartment. Remove hose and obtain a fixture extender or a shorter food grade hose to prevent cross-connection.

Repeat: RFE does not comply with Level One Certification requirements. No Person-in-Charge food handler certification on-site at the time of inspection.

• Winan’s, 1201 Experiment Farm Road, Troy — Repeat: Outer opening not protected. Observed gaps at the top and bottom of the back door. Observe light through gaps. Fix or replace door sweeps to eliminate possible point of entry for pests.

Repeat: Observed a box of clear single use cups stored on the floor in the back by the 3 compartment sink. Single use articles must not be stored on the floor, ensure at articles are stored at least 6 inches off the ground.

Repeat: Non-food contact surfaces of equipment are unclean. Observed large amounts of coffee grounds build-up on, under and around coffee brewer.

Repeat: Water treatment device or backflow preventer not inspected/serviced with documentation as required. Observed inspection tag has not been updated in the past year. Ensure backflow inspections are up to date and keep records.

Repeat: Plumbing system not properly maintained or repaired. Observed grease trap lid not maintained or in good repair.

Repeat: Physical facilities not maintained in good repair. Observed counter top underneath espresso machine is lifting creating an area that is not easily cleanable. Observed coffee grounds underneath lifted section of counter.

Water treatment device or backflow preventer not inspected/serviced with documentation as required. Observed backflow preventer inspection due in March, tag does not show inspection. PIC stated it has been inspected, discussed with PIC keeping documentation of inspections.
