May 20

• Miami Valley Wine & Spirits, 943 W. Main St., Tipp City — Outer opening not protected. Observed back delivery door with a gap allowing sunlight to enter and entry point for pests to enter the facility. Outer openings should be closed and tight-fitting to protect against entry of pests.

Equipment components are not intact, tight or properly adjusted. Observed gasket on right beer cave retail cooler door to be in disrepair and repaired with white duct tape. Gasket was not allowing cooler door to seal shut. Please repair and replace.

Warewashing equipment and/or components not cleaned at required frequency. Observed 3-comp sink inaccessible and filled with unnecessary items and clutter. Remove items and clean so 3-comp sink is readily available if needed within the establishment.

Facility not maintained clean. Observed beer cooler with build up of dust, dirt and other debris on the ground and underneath racking/shelving units. Clean beer cave at an ample frequency to prevent the build up of unnecessary dust, dirt and debris.

• Menards Inc., 75 Weller Drive, Tipp City — Facility does not have an employee with manager certification in food protection. There is not an employee with level 1 food safety training.

Handwashing sign(s) not posted. Observed women’s restroom does not have a sign stating employees must wash hands before returning to work. PIC stated he would get one for that restroom.

Outer opening not protected. Observed sizeable gap between outer doors leading into the stock yard from food over stock area. Openings observed are large enough to admit pests. PIC was shown doors in violation.

• Dollar General Store, 965 S. Miami St., West Milton — Corrected During Inspection; Critical: Did not require employee(s) to report symptoms, diagnosed illness by a health care provider, or high risk situations as specified in rule. At time of inspection there was not an Employee Illness Reporting document on file. PIC was given a copy of Employee Reportable Illness document to have all employees read and sign at time of inspection.

Corrected During Inspection; Critical: Food not properly protected from contamination by separation, packaging, and segregation. Observed raw bacon being stored above hot dogs which are a ready to eat food. PIC corrected violation at time of inspection by moving the raw bacon to the shelf below the hot dogs.

May 21

• Wellness 360, 1845 W. Main St., Troy — Corrected During Inspection; Critical: Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed cut leafy greens in undercounter reach in cooler without a date mark. Discussed once Time/Temperature Controlled for safety product has been “opened” or “prepped,” it shall be dated for 7 days with the day of preparation counting as day one. Food employee properly date marked at the time of inspection.

Corrected During Inspection: Non-food contact surfaces of equipment are unclean. Observed residual liquid in the bottom of the undercounter reach in refrigerator unit. Discussed non-food contact surfaces of equipment shall be clean to sight and touch. Food employee removed all food items and cleaned unit at the time of inspection.

• Ullery Concessions LLC, 7960 Frederick-Garland Road, Union — Critical: Private water system not sampled or tested as required. Discussed mobile will need to have well water sampled and tested if using to wash, rinse and sanitize food contact surfaces of equipment/utensils or using water for event not allowing hook-ins.

• Gustin Concessions 2, 5055 E. Snyder Road, Fletcher — No violations were documented at the time of inspection.

• Gustin Concessions 1, 5055 E. Snyder Road, Fletcher — Corrected During Inspection: Cleaning agents and/or sanitizers not available in the mobile food service operation. Adequate EPA registered sanitizer not available at the time of inspection. PIC obtained registered chlorine bleach sanitizing solution from other trailer at the time of inspection.

May 22

• Fire Pie Catering, 3050 Tipp-Cowlesville Road, Tipp City — Critical: Employees are not informed in a verifiable manner of their responsibility to report information about their health. No health policy on-site at the time of inspection.

No written procedures for responding to vomiting or diarrheal events on-site at the time of inspection. Discussed procedures for responding to boidly fluid accidents shall be kept on the mobile FSO in response to an event calling for step-by-step procedures for appropriately cleaning and disinfecting potentially hazardous bodily fluids.

• Kountry Kruise Thru, 7235 State Route 36, Fletcher — Facility does not have an employee with manager certification in food protection. PIC notified.

• Big Mike’s Gas N Go LLC, 1333 Archer Drive, Troy — No violations were documented at the time of inspection.

May 26

• Meijer Distribution Building 804 Mkt. C, 4200 S. County Road 25A, Tipp City — Equipment components are not intact, tight or properly adjusted. Retail freezer light not working properly at the time of inspetion. Please fix and email photo of properly adjusted internal LED shielded lighting fixture to freezer case.

• Meijer Distribution 802 Remote Mkt. C — No violations were documented at the time of inspection.

• Meijer Distribution Building 805 — Corrected During Inspection; Critical: Ready-to-eat, TCS food not properly discarded when required. Observed TCS pre-packaged food items past date of dispostion at the time of inspection. Account Manager/Person-in-Charge removed items for food employee to scan out for credit at the time of inspection.

• Meijer Distribution Building 802 — No violations were documented at the time of inspection.

• Meijer Distribution Building 801 — No violations were documented at the time of inspection.

• Dollar General, 1931 Covington Ave., Piqua — Repeat: Equipment components are not intact, tight or properly adjusted. Observed front reach in freezer with condensation all over the front and inside of the doors. Observed ice build-up on the top of the freezer door.

Repeat: Floors, walls, and/or ceilings not smooth and easily cleanable. Observed a large crater line in floor in the front by the reach in coolers. Cannot be easily cleaned.

• BP, 1590 W. Main St., Troy — No violations were documented at the time of inspection.

• Igloo Drive Thru, 945 W. Main St., Tipp City — Facility does not have an employee with manager certification in food protection.

Plumbing fixtures unclean. Observed build-up on sink counter, floors and toilet.

May 27

• Hobart Westbrook Market C, 101 Trade Square, Troy — No violations were documented at the time of inspection.

• Hobart Institute Market C, 400 E. Trade Square, Troy — Non-food contact surfaces of equipment observed unclean at time of inspection. Observed severe ice build up in freezer case. Discussed non-food contact surfaces of equipment shall be kept free of an accumulation of debris such as ice contamination.

• Abbott Nutrition Market C, 1 Abbott Parkway, Tipp City — No violations were documented at the time of inspection.

• Cedar Springs Pavilion, 7931 County Road 25A, Tipp City — No written procedures for responding to vomiting or diarrheal events. Only observed spill kit assembly page of bodily fluid clean up procedures on-site at the time of inspection.

Irreversible registering temperature indicator not readily accessible for hot water mechanical warewashing operations. No heat strips for high temp dish machine. Person-in-Charge discussed obtaining high temp dish machine thermometer before opening for seasonal banquets/events.

• Whole Health For The Whole Family, 22 S. Weston Road, Troy — Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. Observed missing thermometer in freezer unit. PIC was notified. PIC believes it could be in the back of the unit and will look for it.

• Izzy’s Drive Thru, 7500 State Route 571, West Milton — Repeat: Facility does not have an employee with manager certification in food protection. At time of inspection, a copy of an employee level 1 food safety training certificate was not available.

Repeat: Outer opening not protected. Observed door labeled “customer entrance” has a large gap between the bottom of the door and the ground. Opening provides possible entrance for pests.

Repeat: Improper use or placement of insect control devices. Observed rodent glue traps underneath chip racks in drive thru area. Glue traps cannot be used in food facilities. PIC was notified that traps must be removed.

Repeat: Physical facilities not maintained in good repair. Observed a leak in the back store room ceiling. Observed water damaged ceiling in addition to a catch bucket under the leak. PIC stated that roof is scheduled to be repaired.

Repeat: Facility not maintained clean. Employee restroom and hand sink observed to have dust/debris build up.

• CVS Pharmacy, 804 W. Main St., Troy — Repeat: Outer opening not protected. Observed gaps between the shipping garage door and the floor in the back stock room. PIC stated that it has been addressed before, and will be readdressed. Observed opening underneath back door in the stock room. Door does not meet the ground, PIC was notified.

June 3

• Casey’s General Store, 200 N. High St., Covington — Repeat: No written procedures for responding to vomiting or diarrheal events. At time of inspection PIC was unable to provide documentation for responding to bodily fluid incidents.

Repeat: No towels or drying device at the handwashing sink(s). Observed hand towel dispenser empty and broken. PIC stated it has been broken for an extended time.

Repeat: Equipment components are not intact, tight or properly adjusted. Observed vegetable wash dispensing unit not properly adjusted. PIC stated that maintenance on the unit will be addressed.

Repeat: Non-food contact surface(s) not cleaned at the required frequency. Build up observed on several nonfood contact surfaces throughout kitchen/food prep areas. Nonfood contact surfaces to be cleaned more frequently to prevent build up.

Repeat: Facility not maintained clean. Observed several areas unclean: bag in box area, walk-in cooler, walk-in freezer, kitchen, chemical storage area, and men’s restroom. Observed unclean floors with build up throughout.

• Walmart, 1801 W. Main St., Troy — Repeat: Outer opening not protected. Observed side delivery door through retail area double doors with an inadequate door sweep. Please replace or repair to protect against entry of pests.

Repeat: Non-food contact surfaces of equipment are unclean. Observed gaskets of deli case doors to have a build up of food debris. Several non-food contact surfaces throughout the fresh food area observed not clean to sight and touch.

Repeat: Water treatment device or backflow preventer not inspected/serviced with documentation as required. Observed backflow preventer in dept. 80 not serviced since 2018. Verifiy with state ceritifed plumber registered in Miami County and have backflow serviced. Backflows shall be serviced annually.

Physical facilities not maintained in good repair. Observed produce walk-in cooler condensing fan not in good repair. Observed an active leak in unit. Also observed a leak in the walk-in cooler meat room condensing unit. Both units were leaking and collecting onto the ground in the walk-ins. Please repair.

Repeat: Physical facilities not maintained in good repair. Observed broken floor tiles in walk-in freezer of the bakery, missing cove tile base in deli dept., and hole in the wall exiting the bakery and heading into the meat area. Physical facilities shall be maintained in good repair.

June 4

• Meijer Inc. Dairy Market C, 4200 S. County Road 25A, Tipp City — No violations were documented at the time of inspection.

• Dolphin Club of Troy, 2326 State Route 718, Troy — Device used to generate sanitizer onsite does not comply with EPA regulations or manufacturers instructions. Observed a bleach being used that is not EPA registered for food contact surfaces.

Warewashing equipment and/or components not cleaned at required frequency. Observed build-up in third compartment of three compartment sink.

Non-food contact surfaces of equipment are unclean. Observed splatter residual on the ceiling of the microwave oven.

June 5

• Marmee’s Pantry, 25 S. Miami St., West Milton — No violations were documented at the time of inspection.

• The Cellar, 1600 W. Main St., Suite A, Troy — Outer opening not protected. Observed light beneath the back exit door. PIC was notified it is a possible entrance for pests.

• Burger King, 1408 Covington Ave., Piqua — Violations/comments: The following food contact surfaces need cleaning: food containers and microwave interior. The floors need cleaning mainly by fryer. Monitor times closely for items that are using time as a public health control i.e. cut tomatoes, cut lettuce, etc.

May 27

• Rally’s, 118 N. College St. Piqua — Violations/comments: Observed food containers that needed to be cleaned. Wash, rinse and sanitize. Holes in walls. The walls must be smooth, durable and easily cleanable.

• Piqua Junior Baseball and Softball, P.O. Box 1841, Piqua — Violations/comments: Satisfactory at time of inspection. Use metal item thermometer to monitor food temps. Use test strips to monitor strength of sanitizer.
